AIDS Care Group is pleased to announce its selection as a participant in a Special Project of National Significance grant to conduct case finding and help link patients into durable HIV/AIDS care. Seeking to address patients who have unmet HIV healthcare needs within Southeastern Pennsylvania, the SPNS project hopes to identify best practice concepts to help reduce the nation’s estimated 222,000 persons who are living with, but not linked into durable HIV healthcare. With an emphasis on working within minority communities and acknowledging that patients will most likely need a multidisciplinary approach to both healthcare and social determinants of health, AIDS Care Group will seek support from jurisdictional leadership within the HIV/AIDS provider community, community partners in healthcare and social services, members of the State and county correctional systems, and providers of care for those with substance use disorders and/or mental health problems. With funding from the Minority AIDS Initiative and Part F Special Projects of National Significance, this four year project is identified as a System-Wide Syndemic Approach to removing the remaining barriers to achieving an end to the HIV epidemic.
